Özet
Premenstrual dysphoric disorder (PDD) is characterized by mental, physical and cognitive symptoms that occurs in the late luteal phase of the menstrual cycle and regresses in the week following menstruation. In PDD, serotonin reuptake inhibitors and combined contraceptives are the primary pharmacologic treatments. In cases where there is a personal or family history of bipolar disorder (BD), the use of antidepressants may pose a risk of inducing manic episodes. The frequent coexistence of BD and PDD, the fact that both diseases are cyclic in nature and that common mechanisms such as hormonal changes play a role in their aetiologies, suggest that lithium might be efficacious in the treatment of PDD. Here, we present a case who didn’t have a BD but a family history of BD and was treated with lithium monotherapy for PDD with a successful outcome. In cases where first- and second-line therapies cannot be used or no response is obtained in PDD patients, pharmacological agents that have demonstrated efficacy in preventing mood episodes among first-degree relatives, may present a viable solution

Epworth Sleepiness Scale and Polysomnographic Evaluation of Dysthymic Women with Chronic Insomnia

Objective: In patients with dysthymic disorder (DD), sleep problems are assessed as a part of their depressive state, and different treatable conditions, such as other primary sleep disorders, are often skipped. We aimed to determine symptoms related to daytime sleepiness and polysomnographic findings in dysthymic women with chronic insomnia and to find out whether or not there is an accompanying sleep disorder. We suggest that pathologies that can lead to sleep problems in dysthymic patients should be searched, and to this end, application of the Epworth sleepiness scale (ESS) and polysomnography examination should be used much more frequently. Patients and Methods: We included 20 female dysthymic patients with complaints of chronic sleep problems who had been under follow-up at Uludag University Hospital Psychiatry Outpatient Clinic for at least the last two years. Twenty healthy female volunteers, aged 18-65 years, were included in the study as the control group. ESS and polysomnography examination were applied in all patients. Results: When patients with DD and healthy controls were compared, significant differences in ESS scores and ratios of sleep phases were determined between groups. DD patients had higher ESS scores and higher ratios of rapid eye movements (REM), non-REM-1 (Stage 1), non-REM-2 (Stage 2) phases and lower ratios of slow wave sleep (Stage 3 + Stage 4). Conclusion: The findings of our study suggest that there are structural sleep changes in dysthymic patients, but no direct connection between sleep variables and depressive states is evident.

Özet
Neurofibromatosis type 1 is a common neurogenetic disorder. Psychiatric disorders are more common in patients with neurofibromatosis type 1 than the general population. Here we report a neurofibromatosis type 1 patient associated with generalised anxiety disorder. Although the patient is under psychiatric follow-up for approximately 15 years her dermatological complaints has been ignored. Because of this, dermatological lesions that are persisting over 40 years, the patient did not diagnosed as Neurofibromatosis type 1, in case social, pyshcologic and biologic complaints associated with Neurofibromatosis type 1 is ignored. It is clear that the history of the patient was not understood as a whole and she was exposed to use of antipsychotics in an inappropriate way. Because of using antipsychotics tardive dyskinesia occured in this patient and this reminds us to be more careful while prescribing medicines. It should be noted that human is a biopsychosocial entity and biologic, psychologic and social factors play role during the illness process. Physcians should evaluate their patients as a whole, not only their complaints and family history for not missing any medical condition.
